Concierge Geriatric Care Management Services

Comprehensive assessments to assist families in determining the best course of action, whether it is home care with agency services and care management, or placement in the most appropriate facility. Assessments include social, psychological, medical, legal, environmental, and financial aspects of a person’s life, providing a detailed Plan of Care.

  • Assist with moving to or from a retirement communities, care home, or nursing home

  • Attend doctor appointments & facilitate communication between provider client/family

  • Consumer education & advocacy

  • Counseling & support

  • Crisis intervention - navigate through emergencies, hospitalizations, rehab stays

  • Liaison to families at a distance, making sure things are going well and alerting families to problems

  • Screen, arrange & monitor in-home help or other services

  • Witness in probate court and serve as Legal Guardian

    *Hourly bill rate based on assessment.

Details

ASSESSMENT AND PLAN OF CARE DEVELOPMENT - Assessment interview (including: medical, cognitive, social, functional, safety, legal, basic review of financial resources), creation of individualized Plan of Care. EMAILS - Time accrued while writing an email and sending to client or to any other entity related to the CGI agreed upon services for each client.

LEGAL/COURT PREPARATION, COURT APPEARANCE, EXPERT WITNESS - Time spent for the purpose of or in connection with court proceedings, including but not limited to: telephone calls, meetings, and written communication to/from client, family, physicians, attorneys or any other person involved in the client’s care, preparation of expert reports and affidavits, assistance in the response to court discovery, depositions, trial preparations, trail attendance and testimony.

ONGOING CARE MANAGEMENT - All time spent performing care management services, including but not limited to: telephone calls to/from client, family, physicians, attorneys or any other person involved in the client’s care; medication refills and set-up; updating the client profile, attended physician appointments; ongoing personal visits at the home or elsewhere, and drive time necessary in the course of care management.

MISCELLANEOUS EXPENSES - Costs and expenses advanced, such as item purchases, photocopy, and delivery charges, will be itemized and billed. Expense invoices will be sent with monthly bill.

TRAVEL TIME/EXPENSES/MILEAGE - Drive time to and from client from Care Manager’s home office to client home, hospital, appointments, shopping, errands or other tasks that involve driving.
